COVIDCare@Home is a program designed to support primary care providers to care for their patients with COVID-19 while keeping them safely at home. If you have been diagnosed with COVID-19 or are presumed to have COVID-19 and live in Toronto, talk to your primary care provider about our program.
If you think you have COVID-19 symptoms or have been in close contact with someone who has it, use our pre-assessment tool to determine if you need to seek further care.
