COVIDCare@Home is a program designed to support primary care providers in Toronto to care for their patients with COVID-19 while keeping them safely at home.
Services:
- Primary care providers who are monitoring patients with COVID-19 will have access to a real-time virtual support hub, housed at Women’s College Hospital, providing expertise in general internal medicine, nursing, social work, pharmacology, and psychiatry
- Primary care providers who would like support in monitoring their patients with COVID-19 will have access to a team of family medicine residents, staff physicians, and internists who will take the lead on remote monitoring, with meaningful handover and ongoing communication between WCH and the PCP

If you have a patient who has COVID-19 symptoms or who has been in close contact with someone who has it, please direct them to our pre-assessment tool before sending them to our assessment centre.
